SMS Privacy Policy

1. Introduction

Precision Facial Surgery ("we," "us," or "our") respects your privacy and is committed to protecting your personal information. This SMS Privacy Policy describes how we collect, use, and protect information in connection with our text messaging (SMS/MMS) communications program. Message frequency varies and message and data rates may apply. This policy works alongside our SMS Terms & Conditions.

2. Information We Collect

When you opt in to our SMS program, we collect:

• Your mobile phone number
• Your name (as provided during patient registration)
• Your consent status and the date/time consent was provided
• Message content you send to us via text
• Message delivery status and timestamps

3. How We Use Your Information

We use the information collected through our SMS program to:

• Send post-operative check-in messages following surgical procedures
• Respond to your text messages regarding your care
• Facilitate communication between you and your surgical care team
• Maintain records of communications as part of your medical chart

We do not use your phone number or SMS data for marketing purposes. Our SMS program is limited to clinical care communications.

4. Information Sharing

We will not share your opt-in to an SMS campaign with any third party for purposes unrelated to providing you with the services of that campaign. We do not sell, rent, or share your phone number or SMS message content with third parties for their marketing purposes. We may share information with:

• Our SMS service provider (Notifyre) solely for the purpose of delivering messages, subject to their own privacy policy and our Business Associate Agreement
• As required by law, regulation, or legal process

All SMS communications are stored as part of your protected health information (PHI) and are subject to the same privacy protections as your medical records under HIPAA.

5. Data Retention

SMS message records are retained as part of your medical record in accordance with applicable state and federal medical records retention requirements. Your phone number and consent status are retained for as long as you remain an active patient or until you opt out of the SMS program.

6. Opt-Out

You may opt out of our SMS program at any time by replying STOP to any message from us. Upon opting out, you will receive a confirmation message and no further text messages will be sent. Opting out of SMS does not affect your ability to receive care from our practice.

8. Security

We implement reasonable administrative, technical, and physical safeguards to protect your information. All message data is encrypted in transit and at rest. Access to patient communication records is restricted to authorized clinical staff.

We may update this policy from time to time. The effective date at the top of this page indicates when the policy was last revised. Continued participation in our SMS program after changes constitutes acceptance of the updated policy.
